Dependent Verification
What is dependent verification?
To continue coverage for enrolled dependents, EWHT requires documents that show your dependent’s relationship to you. Even if you’ve already provided these documents to your employer, you must submit dependent verification to EWHT within 90 days of your employer notifying EWHT of your dependent’s enrollment. This process ensures that only eligible dependents are covered under the plan.
If you received a mailed letter from EWHT, please follow the specific deadline written in that notice. You may also find message requests in your EWHT Member Portal.
Required Documents by Dependent Type:
The documents required depend on the type of dependent enrolled. Please submit copies of the applicable documents listed below.
For a spouse:
- A copy of your certified record of marriage (sometimes called marriage certificate or marriage license), that’s been issued from your state, county, federally recognized tribal government office, or similar authority, showing the filed application, license, and record of marriage. Any document with “commemorative,” “heirloom,” or “for display purposes only” cannot be used as legal proof of marriage.
- And a copy of their Social Security Card or Individual Taxpayer Identification Number (ITIN).
- If you are no longer married to a spouse who is enrolled in the health plan, submit a copy of your divorce decree(s).
For domestic partnerships:
- Either a state-issued certificate of domestic partnership, or
- A signed EWHT domestic partnership affidavit, which you can download here.
- And a copy of their Social Security Card or Individual Taxpayer Identification Number (ITIN).
For children and stepchildren, submit one of the following:
- A copy of the birth certificate, adoption decree, legal guardianship order
- Qualified Medical Child Support Order (QMCSO)
- National Medical Support Order (NMSO)
- Other court order proving the relationship between you (the primary subscriber), and your dependent(s).
- And a copy of their Social Security Card or Individual Taxpayer Identification Number (ITIN).
Identification and name changes:
- Include the Social Security number (SSN), or Individual Taxpayer Identification Number (ITIN), or other government-issued identification for dependents over age one.
- Legal documentation for any name changes such as your child’s marriage certificate or court documents.
Submit scanned copies, photocopies, or legible photos of verification documents to EWHT.
If the document is 2-sided or has multiple pages, make sure you submit all pages and both sides of the paper. Submit the documents one of these ways:
- Upload to the EWHT Member Portal (the fastest and most secure method).
- Email EssentialWorker@RISEpartnership.com. Please include your name and the last 4 digits of your Social Security number in the body of the email.
- To mail, send copies of your documents to:

Please include your name and the last 4 digits of your Social Security number on each document.
If we do not receive the required verification documents for the dependents on your plan, their eligibility for EWHT benefits will end. You may also be responsible for paying your dependent’s previous medical and prescription costs incurred on the EWHT plan.
What happens after you submit dependent verification documents?
EWHT will typically review your documents within 3 business days and contact you via member portal message if more information is needed.
To confirm that your documents have been received and approved, please check your EWHT Member Portal messages or call customer service at (833) 389-0027, Monday- Friday from 8 AM to 5 PM.
