Dependent Verification
What is dependent verification?
EWHT requires documentation proving your dependent’s relationship to you. You may have done this for your employer, but you must also submit dependent verification to EWHT within 90 days of the start of coverage.
Required Dependent Verification Documents:

- For a spouse: submit a copy of your marriage certificate. If you are no longer married to a spouse who is enrolled in the healthcare plan, submit a copy of your divorce decree(s).
- For domestic partnerships: submit either a state-issued certificate of domestic partnership or a signed EWHT domestic partnership affidavit, which you can download here.
- For children submit one of the following:
- A copy of the birth certificate, adoption decree, legal guardianship order
- Qualified Medical Child Support Order
- National Medical Support Order
- Other court order documentation that proves the relationship between you, the primary subscriber, and your dependent(s).
Be sure to include legal documentation of any name changes, such as your child’s marriage certificate or court documents, as well as the Social Security number (SSN). If your dependent doesn’t have an SSN, submit another government-issued identification for all dependents over the age of one.
Submit scanned copies, photocopies, or legible photos of verification documents to EWHT.
If the document is 2-sided or has multiple pages, make sure you submit all pages and both sides of the paper. Submit the documents one of these ways:
- Upload to the EWHT Member Portal.
- Email EssentialWorker@RISEpartnership.com. Please include your name and the last 4 digits of your Social Security number on each document.
- To mail, send copies of your documents to:

If we do not receive the required verification documents for dependents on your plan, their eligibility for EWHT benefits will end. This means you may be responsible for reimbursing medical and prescription costs.
What happens after you submit dependent verification documents?
EWHT will typically review your documents within 3 business days and contact you via member portal message if more information is needed.
To confirm that your documents have been received and approved, please check your EWHT Member Portal messages or call customer service at (833) 389-0027, Monday- Friday from 8 a.m. to 5 p.m.
