Dependent Verification

What is Dependent Verification? 

This means providing documents that show your family member is related to you. You may have done this for your employer, but you must also submit them to EWHT within 90 days of your dependent’s coverage start date. You can submit legible photos, scanned copies, or photocopies.

Submit scanned copies or legible photos of the documents to the EWHT.

If the document is two-sided or has multiple pages, make sure you submit all pages and both sides of the paper. Submit the documents one of these ways: 

  1. Upload to the Essential Worker Member Portal.

     

  2. Email essentialworker@RISEpartnership.com. Please include your name and the last 4 digits of your Social Security number on each document.

     

  3. To mail, send copies of your documents to EWHT at PO Box 94392, Seattle, WA 98124. Please include your name and the last 4 digits of your Social Security number on each document. 

If we do not receive the supporting documents for dependents on your plan, their eligibility for EWHT benefits will end. This means you may be responsible for reimbursing medical and prescription costs. 

What happens after you submit dependent verification documents?  

EWHT will review your documents and will reach out if we need more information. 

    1. If you submit by uploading pictures or copies of your documents, log in after 3 days to see if they have been accepted.

    2. If you send them by email, someone from EWHT will get back to you. Keep an eye out for an email from MagnaCare.com.

    3. If you send them by mail, you can contact customer service at (833)-389-0027 Monday – Friday during normal business hours.
